| INTRODUCTION TO EMPLOYEE ASSISTANCE PROGRAM Dear Employee and Family Members: The purpose of this information and video is to introduce your EAP, Community Action EAP Inc. (CA/EAP) to you and your family members. Your Employee Assistance Program (EAP) is a service program which is being provided to you by your employer. Your employer offers special assistance through the EAP to employees and family members desiring confidential help in resolving personal issues. The EAP seeks to help people resolve problems before they become overwhelming, adversely affecting one's life at work and at home. The Employee Assistance Program can help people resolve personal issues, such as, marital/relationship or family conflict; emotional distress; alcohol, drugs or chemical dependency; financial issues, or legal concerns, etc. EAP services are provided at no cost to you and include an initial assessment session which determines whether the issue can be resolved in the EAP or requires referral to ongoing assistance. If ongoing professional help is necessary, the EAP will offer you affordable options as referrals, providing professionals and resources through one’s insurance and/or alternatives which can help to minimize the cost. As a support to you, you have access to the most current mental/behavioral health information through CA/EAP’s On-line Wellness Library, accessible from the Home page of this site. Your employer will convey to you your organization’s Log-in and Password which is required to access the On-line Wellness Library. Limits of Confidentiality: Confidentiality is maintained in accordance with law. Client information will be disclosed without the client's written consent in the following cases: (1) when reporting child abuse and/or elder abuse and/or dependent adult abuse, (2) when relating information about being a risk to oneself or others, (3) when relating the intention to commit illegal acts or threats to the organization, other employees, or the EAP, and (4) when a supervisory referral is made by a supervisor, information that the employee contacted the EAP and attended educational/assessment session(s) will be communicated by the EAP to the designated supervisor. Any member of the family: the employee, spouse, children, significant other, can receive help. Simply call the toll-free, 24-hour Help Line at 1-800-777-9376 to receive assistance. When you call, you will need to give basic information so we can provide assistance to you. For internal administrative purposes of the EAP in helping you access insurance benefits and alternative resources as needed, the basic information includes one’s social security number and the date of birth. Be assured that all basic information is within the limits of confidentiality and is only collected voluntarily from you. Remember your EAP will be able to serve you beneficially with or without this specific information. Please remember that the hope of the EAP is to intervene early in personal issues before they evolve into situations of severe distress.
